Instructions For Issuance of Domicile Certificate

In order to obtain a domicile certificate, an individual / Applicant must provide the following documents: 1. Domicile Certificate must be verified & attested by:

i. Gazetted Officer/Union Council Secretary /General Counselor.

ii. Patwari

iii. Kanungo

iv. NaibTehsildar

v. Assistant/Additional Assistant Commissiner

vi. Deputy Commissioner

2. Two (2) Passport size photographs front side attested from Gazetted Officer / Union Council Secretary /General Counselor. 3. Original & Copy of Applicant CNIC or Form ‘B’ for under 18 years of age. 4. Original & Copy of Parent’s/ Guardian’s CNIC. 5. Applicant must be available personally at the submission time of Domicile Certificate. 6. Mobile number must be written on the back side of Domicile Certificate.
